Simple Addiction Surveys (in PHP)

These are very simple Yes/No survey's, similar to the "Addicted to AIM" quiz. Feel free to share the URL, paste the results to your web site, blogs, profiles, etc. If you decide to implement your own version, no permission is required, though I'd appreciate a quick email. That doesn't mean copying the questions verbatim as some sites have done. That's called stealing. If you want to host a survey here, just create a text file with a similar structure to example.phps and e-mail it to contact @ moundalexis.com

Notes

To PHP Folks -- Each survey consists of four items. The topic variable, credits line, and an array of questions. The script then includes guts.php to do the work. You can view the source, guts.phps. That's it.

To Counterstrike Kids -- I host the survey. I don't play CS, host CS, or like CS... not my cup of tea. Quit messaging me about it. Did you know that the Counterstrike surveys are requested about 20-30,000 times per month? You kids are scary with your forums.
